CO₂ Laser Surgery for Ovarian Endometriosis and Fertility Preservation

Epidemiological studies show a link between endometriosis and infertility. Infertility evaluations are a common reason for the diagnosis of endometriosis, and infertility is one of the possible consequences of endometriosis.
It is estimated that 20 to 50% of patients seeking treatment for infertility have endometriosis, and that 30 to 40% of patients with endometriosis also have infertility issues.

The CEC has incorporated this aspect into its core mission since [year]. Our team has published numerous scientific articles on the subject, and we are actively working to confirm the benefits of surgery in this specific context. When performed properly, surgery can preserve ovarian function and increase the chances of conception.

Why is the use of CO₂ lasers in endometriosis surgery by top-tier surgeons unique in France?

The CEC has extensive and recognized experience in the use of the CO2 laser (Acupulse, Lumenis) in laparoscopic surgery, having performed more than 5,000 surgeries for deep endometriosis using this technique. Currently, the CEC is the only center in France offering laparoscopic CO2 laser surgery for the treatment of endometriosis in its most complex forms, with experience documented in the scientific literature.
The CO2 laser has proven highly effective in the conservative treatment of ovarian, gastrointestinal, and urological endometriosis, and offers numerous advantages. It allows for highly precise dissection, and its use is associated with a lower risk of bleeding as well as faster recovery, as evidenced by an average hospital stay of 1 to 2 days for deep rectal endometriosis.

Augmented reality with SURGAR

We had the opportunity to offer our patients a preview of this type of tool, as part of a scientific study.

SURGAR is the first augmented reality software to support laparoscopic surgery. It enhances laparoscopic surgical precision by merging the surgical view with a real-time digital twin.

The use of augmented reality is the next step in minimally invasive surgery.

It's been a long time since we've had the impression of a major step forward in surgery. As one of the first to use this software, we can see that it opens up a wide range of possibilities for minimizing the effects of surgery.

More training courses

The Complex Endometriosis Center reaffirms its commitment to improving the daily lives of women suffering from endometriosis, and one of its concrete actions is to raise awareness among local healthcare professionals.

The aim is to make progress in terms of diagnosis and appropriate patient management.

For almost 20 years, Professor Olivier DONNEZ and his teams have been working at national and international level to improve understanding of the disease and optimize care.

To this end, the EDUPRAT Endometriosis face-to-face training course on September 19, 2024 and the Manoléta Endometriosis training course on September 21, 2024 in Avignon brought together many professionals in the field.

Our coordinator answered patients' questions directly at a round table discussion on September 28, 2024 in Valréas.

Surgery for multiple digestive endometriosis lesions

Endosud network RMM meeting presentation June 4, 2024

The CEC regularly participates in the Endosud network as part of the establishment of a specialized endometriosis care pathway in the PACA region and presents the results of its latest studies.

Professor DONNEZ presented the benefits of shaving in combination with short sigmoid resection for patients with multiple lesions, as well as the long-term outcomes for these patients. These procedures are routinely performed at the CEC in close collaboration with the Department of Gastrointestinal Surgery and Dr. Alexandre LlORY.

The undeniable advantage of this technique is that it preserves the rectum intact (rectal shaving) while performing a short intestinal resection averaging between 4 and 7 cm. The colonic anastomosis is therefore located higher up, away from the Douglas pouch, which reduces the risk of vaginal fistula since there is no rectal resection and thus no low anastomosis. The average hospital stay is 3 to 5 days, and the risk of complications is as low as for conservative surgery, while maintaining a very low recurrence rate of approximately 1.6%.

The CEC is currently a pioneer in this surgical technique, in which shaving is combined with gastrointestinal resection for the first time.

We're making progress

The CEC is proud to have participated in this research, which is now being applied to scientific studies on new molecules for the treatment of endometriosis.

This is a continuation of scientific work on a GnRH antagonist, Linzagolix, which CEC has already offered its patients in a pilot trial in 2018.

At 3 months, Linzagolix administration showed a clinically and statistically significant efficacy in reducing dysmenorhoea and intermenstrual pain.

The proportion of patients responding to dysmenorrhea was 73% in the treated group versus 23% in the placebo group.

This is a significant advance in the medical treatment of women suffering from endometriosis.

Presentation on March 6, 2024 in London on Shaving

On March 6, 2024, Professor DONNEZ took part as a renowned surgeon in the surgical management of endometriosis. He presented the London team with his knowledge of the surgical technique of shaving and the results of the numerous publications on the subject.

Invited by the London-based Centre for Endometriosis and Minimally Invasive Gynaecology (CEMIG), discussions at the conference led to advances in the surgical management of the condition.

Endometriosis is a pathology that knows no borders, and international collaboration between scientists is important.

The World Health Organization estimates that 190 million women are affected.

The CEC is taking concrete action on the international stage.
